Service Coordinator – Third Sector Link Worker
Location: Wrexham 
Contract type: FTC until 31/3/2020
35 hours per week
Salary: GBP 18,805 per annum

About the role
This is a dynamic front line position, located with a multi-disciplinary team of health and social care professionals in the Wrexham Single Point of Access.  
The Third Sector Link Worker plays a crucial role in the Wrexham Single Point of Access (WSPoA) with respect to the provision of information, advice and assistance. This Third Sector perspective within the Wrexham Single Point of Access supports health and social care professionals as well as the citizens to achieve their goals in line with the strategic aims of Social Services and Wellbeing (Wales) Act 2014.  The job holder will achieve this in a number of different ways including by managing and working with a caseload of allocated individual citizens to develop alternative community based packages of care utilising the “what matters” asset based conversation approach. This to be completed over the phone and or by visiting identified cases allocated to caseload.
